ANYONE concerned about a child or teen’s behaviors, choices or mental health can refer to YSB.

This includes parents, caregivers, doctors, clergy members, schools, social workers, and liaison officers.

Police, County and Court referrals are also common for situations involving law enforcement, but no one has to wait for someone else to tell them to call YSB.

YSB services are professional, affordable and confidential. We accept many insurance policies and also provide a need-based, sliding fee scale.

 

Law Enforcement & School Partners:

Law Enforcement can use this information for submitting a Referral for Diversion services:

  1. Download and type directly into the blank fields to complete the Diversion Referral Form
  2. Inform the family about the YSB referral and provide them with a completed copy of the form. Save a copy for your records.
  3. Submit completed forms electronically to diversion@ysb.net -OR- Fax/Mail a copy to a YSB office.
  4. Include a copy of the Police Report, if applicable.

We offer a range of early-intervention programming, and serve families living in and around Washington County – including Western Wisconsin.

All it takes is a call to one of our YSB Offices. Our caring and knowledgeable staff will help determine which program is right for your situation.
